Cure Staffing, Inc. is seeking a Social Worker for a long-term temporary assignment for the NFHV program.
The New Family Home Visits Initiative (NFHV) builds on the success of the Newborn Home Visiting Program established in 2004 and supports a call to action by the Mayor’s office Taskforce on Racial Inclusion & Equity. The Taskforce on Racial Inclusion & Equity initiative (TRIE) supports NYC neighborhoods across all five boroughs significantly impacted by the COVID-19 pandemic. The NFHV Program supports the citywide health equity goal by identifying maternal health needs during pregnancy or soon after childbirth, preparing families for delivery and a new baby, and connecting families to needed services essential to the well-being of mothers, children, and families.
NFHV social workers assess and follow up and link high-risk families to additional interventions and support. Follow up with clients referred for additional support. Identify resources for program clients and sustain relationships with community partners.
Social Workers are expected to when required, perform home visits to families with newborns. In collaboration with Public Health Advisor Home Visitors, they assess the client and the family, social, environmental and health needs. Promote the well-being of mothers, fathers, caregivers, and their families. Educate and connect families to appropriate referral services. Advocate on behalf of clients when barriers to accessing services exist. Establish, develop, and maintain good relationships between CBO and other City Agencies.
To respond timely and be able to access client needs. Document services are rendered to clients due to record-keeping methods established by the Program. possess the capacity to work with a multi-disciplinary team to ensure client needs are met. Can educate and connect families to appropriate referral services. Ensure referral resources are updated to date and accessible to clients. Have great interpersonal skills to operate as a liaison between NHVP/DOHMH and other agencies that provide services to birthing families.
To work cooperatively with NHVP families by conducting home visits and clinical support during home visits. Support client assessment and referral for appropriate mental health and other social service services. Work in a multidisciplinary team environment to assist families in accessing appropriate referrals. Act a liaison between families and community agencies. Maintain client confidentiality. Enter social worker notes in the database system to document services and encounters between social workers and program clients served.
The Social Worker will be expected to conduct home visits. There will be a MetroCard reimbursement for the travel.
