##  Social Worker  ## 

Location: Syracuse, NY 

Salary: $62.00 / hr 

We are seeking a dedicated and experienced Social Worker to join our team at a leading hospital in Syracuse, NY. This position will provide essential support within the Inpatient Adult Psychiatric Unit, serving adults aged 18 years and older who require short-term, acute inpatient mental health care.

 Qualifications

– L.M.S.W., L.C.S.W., or eligible

– Minimum of 3 years relevant post-master’s experience, preferably in a hospital and/or healthcare environment

– Must successfully pass pre-employment health clearance

 Key Responsibilities

– Conduct comprehensive psycho-social assessments of patients to identify needs and develop appropriate intervention strategies

– Provide supportive counseling and therapeutic interventions to patients and their families

– Collaborate with the multidisciplinary treatment team to create and implement individualized treatment plans

– Assess discharge planning and aftercare needs for at-risk patients, ensuring continuity of care post-discharge

– Address social determinants of health that may impact patient outcomes and well-being

– Facilitate communication between patients, families, and healthcare providers to ensure a holistic approach to care

– Document all assessments, interventions, and patient progress accurately in the medical record

– Participate in departmental meetings, case conferences, and in-service training to enhance professional development

 Required Skills

– Strong interpersonal and communication skills for effective interaction with patients, families, and team members

– Ability to work collaboratively within a multidisciplinary team environment

– Proficiency in crisis intervention techniques and mental health assessments

– Knowledge of community resources available for patient support and aftercare
