Mobile Health Clinician – LPN (New York City)

Licensed Practical Nurses needed for NYC $37.00 per hour
You must be able to interview this week or next week and attend an orientation to be held on Monday 11/18/24 or Monday 11/25/24.
Job Description:
You will be providing our NYC client services in New York City by monitoring, assessing, and providing directed treatment common to complex injuries and illnesses through comprehensive assessments, including diagnosis, treatment, and outcome management for adult and pediatric patients.
Once you are accepted into the Orientation, this orientation will span 2 days. Following the orientation, there will be 3 days of ride-alongs, which do not need to be consecutive, but must be completed within the same week of orientation.
Here is what you can expect day-to-day as a Mobile Health Clinician –NY:
Each day, you will be paired with a driver to travel to patients’ homes to provide care and monitoring .
- While at a patient’s home, you will engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare system
- Throughout the treatment process, maintain patient medical records and documentation consistent with state regulations, our client’s standards, and policy through our HealthPoint and Athena EMR system
- You will be practicing under direction with synchronous telemedicine support
Job Details:
- Candidates with full time availability preferred
- Varying Days in schedule including alternating weekends. Normal hours are 10:00 AM to 8:00 PM.
Responsibilities:
- Act as a conduit for the medical treatment of the Health Plan Partnerships patient population
- Navigate conversations related to management of health, well-being, medication management, and goals of care
- Provide supervised, directed treatment of common medical diagnoses
- Participate in continuing education as required by the state where you reside and the client’s training team
- Adhere to clinical and safety standards, protocols, and performance metrics
- Provide care with cultural sensitivity and compassion
- Lead your practice with a patient-centered approach
- Other related duties as deemed necessary
Qualifications:
- Minimum of two years of experience as a Licensed Practical Nurse
- Active and current American Heart Association BLS Required
- Emergency Department, Urgent Care, Internal Medicine, Hospital – Critical Care, Family Practice or other relevant areas experience preferred
- Current unrestricted license NY State LPN
- Clinical work experience with pediatrics preferred
- Blood Draw/Phlebotomy Certification preferred in addition to on the job experience drawing blood. This will be tested during orientation!
- Fluent in Spanish language preferred
- Ability to lift 50-60 lbs
