Cure Staffing Inc. is seeking two dedicated Community Health Liaison/Case Managers to support critical tuberculosis (TB) prevention and control efforts.

Position Overview

These positions will support critical case management activities, including conducting patient interviews and education, performing care coordination and contact investigations, and facilitating treatment adherence and completion. The liaisons will also be responsible for ongoing communication with healthcare providers and community members, as well as providing directly observed therapy in various settings. Candidates should possess a bachelor’s degree in a health-related field or equivalent experience, along with strong communication and organizational skills.

Position Details

• Schedule: Monday – Friday, 9:00 AM – 5:00 PM (35 hours/week, 1 hour unpaid lunch)
• Location: The Factory, 30-30 47th Ave, Queens, NY 11101
• Pay Rate: 32/hr paid weekly on Tuesdays via direct deposit

Key Responsibilities

• Conduct patient interviews, education, and chart reviews
• Coordinate care and support treatment adherence
• Perform contact investigations
• Document and communicate with healthcare providers and community members
• Support treatment completion through directly observed therapy in various settings
• Assist with case management for newly arrived individuals from high TB incidence countries

Required Qualifications

• Bachelor’s degree from an accredited college, including or supplemented by 12 semester credits in health education or health, social, or biological sciences; OR
• Bachelor’s degree and 6 months of full-time experience in health promotion or disease intervention programs; OR
• High school diploma or equivalent and 4 years of full-time experience as described above

Preferred Skills

• Knowledge of public health or healthcare delivery systems
• Excellent communication and organizational skills
• Proficiency in Microsoft Office suite (Excel, Word)
• Experience working in community-based social services or public health
• Strong writing and verbal communication abilities
• Ability to work independently and as part of a team
• Multitasking capabilities
