Cure Staffing Inc is seeking two dedicated Community Health Liaison/Case Management staff members to support critical tuberculosis (TB) control efforts.

Position Overview

• Location: LIC IN PERSON
• Schedule: Monday-Friday, 9 AM – 5 PM (35 hours/week, 1 hour unpaid lunch)
• Pay Rate: $32/hr

Key Responsibilities

• Conduct patient interviews, education, and chart reviews
• Coordinate care across healthcare providers and community settings
• Perform contact investigations to identify and manage TB exposure
• Support treatment adherence and completion
• Document case management activities
• Communicate with healthcare providers, community members, and other stakeholders
• Provide directly observed therapy in community, shelter, and other settings

Required:

• Baccalaureate degree from an accredited college, with either:
  • 12 semester credits in health education, health, social or biological sciences; or
  • 6 months full-time experience in health promotion or disease intervention programs
• OR: High school diploma with 4 years full-time relevant experience

Preferred:

• Knowledge of public health or healthcare delivery
• Strong communication and organizational skills
• Proficiency in Microsoft Office Suite (Excel, Word)
• Experience working in community social service or public health settings
• Excellent writing and verbal communication skills
• Ability to work both independently and as part of a team
• Strong multitasking skills
